Description

A Community Support Program for Chronically Mentally Ill Persons means a coordinated care and treatment program, which provides a range of treatment, rehabilitation and support services through an identified treatment program and staff to ensure ongoing therapeutic involvement, individualized treatment, rehabilitation and support services in the community for person with chronic mental illness. 

Duration

Up to 2 years with an annual fee.

Instructions

The Division of Quality Assurance sends desk or site renewal information at least 60 days prior to expiration. These materials are to be returned within 30 days of expiration. If you require duplicate information, contact the Division of Quality Assurance, Behavioral Health Certification Section at the number or email above.

* This application should be completed once per certificate annually.
