Description

A Mental Health Day Treatment Program means non-residential, comprehensive, coordinated services to enhance maturation and social development and alleviate a person’s problem related to mental illness providing medical service on a regular basis for a scheduled portion of a 24-hour day including supervised personal care, treatment and psychotherapy in a medically supervised setting.

Duration

Up to 2 years with an annual fee.

Instructions

The Division of Quality Assurance sends Desk or Site renewal information at least 60 days prior to expiration. These materials are to be returned within 30 days of expiration. If you require duplicate information, contact the Division of Quality Assurance, Behavioral Health Certification Section at the number or email above.

The following applications are required for a Mental Health Day Treatment Program recertification:
